This seems to only count from the moment you enter the League until you enter the Hall of Fame and seems to be based on real time because I mostly ran at 6-8x speed but even at 50x speed with an OP Pokemon that oneshots everyone, 17 seconds is an insane time, given that you have to maneuver around the trainers as well. Definitely doable legit tho if this counts the way I assume it does.

I think the leaderboard is based on playtime, as the timer speeds up when you use speedup, and it doesn't really make that much sense for tons of players to spend dozens of hours challenging the Pokemon League. But it seems like it's programmed in such a way that it doesn't account for really high playtimes. I got a time of 26:10 at ~760 hours of playtime (I really like using speedup). If I had to guess, it's probably some sort of overflow issue. So most of the people with really low times probably didn't cheat, they just got "lucky" enough to complete the Elite 4 shortly after the overflow point. It seems like there's a similar issue with the Kanto Speedrun leaderboard too, as my timer went back down to ~10 hours when I tried challenging Red again.
