For the team achievements, the selection screen needs to match this image: http://i.imgur.com/CCCxXBO.png For whatever reason, occasionally it will switch up the bottom three teams in each row. Achievements only work in single player mode.

I fixed the ticket that was filed and looked into the other achievements. There was no password protection at all, lol. You do not need the team selection screen to be a certain way, nor are you restricted from using passwords. As long as you win...
