You can save only in the Yellow Switch and the final castle (although it's best to let the Switch for last 'cause of one achievement). What I did everytime I had just 1 life (but it's a tad slow), was going back to the first stage midpoint and getting many coins over and over, until I get 2 or 3 lives and then carry on.
