This game is not a "prototype" as said in the front page and I think it should be changed into "cancelled" due to license issue. This game officially released on sega genesis in Japan but only few cartridges and few years ago I saw one signed by Alekseï Pajitnov on ebay. Thanks for the achievements by the way !

@Laliluleo: The Genesis Mini version is actually a totally diffefent game. Both it and this version are based on Sega's 1980s arcade Tetris, but the Mini version is a brand new port by M2

Actually, it was indeed cancelled but not completely unreleased. The original version (1989, this one) was slated for release during the Mega Drive's lifespan but was abruptly cancelled due to licensing issues with the Tetris brand at the time. However, it'd get an official release in 2006 as part of the Sega Ages 2500 collection series (more specifically in Vol. 28 as Tetris Collection). The later version (2019, the Mega Drive/Genesis Mini port) is more different and more faithful to the arcade version than this one, so it shouldn't count as the first official release of this port.

Some things are better off not released. This game is a good example. Thanks for the set. This version is pretty horrid but the set was still fun.

The 40k score achievement is cruel and unusual punishment that needs a gambling addict that is adequate in old tetris that does not get bored at boring things to complete.

This is not even the hardest like Acrostiche says one of the easiest just more luck and don't get bored.
Tetris Grandmaster 2 and 2 plus have the most hardest ones with achievements world record players could get with dedication.

I haven't said this is the hardest Tetris version but ONE of the hardest TETRIS SET. Not the same at all, don't let me say what I haven't say :P A Tetris game needing "luck" is what I consider hard, especially when the version is horrid (I'm not the only one thinking that, just check above). The game itself is a poor NES version, not specifically harder. When most of the Tetris sets have some 50-pointer while being more lenient than this version, yes, I still consider this is one of the hardest. You can't reach 40K just by grinding by the way, this becomes really fast at some point.
Check more and more Tetris versions on this site and you won't say this is "one of the easiest" for sure :P
