If you die as the level is ending, and you have to use a Continue to let the level-complete, the achievements for that level will not work. That's an unfortunate currently-unavoidable side-effect from preventing level-skip from popping achievements.

the same, despite not using a continue the achievements still do not work from level 2.

Worked fine for me, makes me wonder if you used level-skip, or you're misinterpreting levels with sublevels.

Right, I was going off of the Sega-CD's level-arrangement, which is the same but doesn't say "Level Complete" when you finish a sublevel, so I had already arranged them this way..
