Yes you can grab green mushrooms. I accidentally hit the one on the string in Forest of Illusion and it still works for part 2. You can also kick that shell in level 1 and start + select out.

grab a beetle shell from the first room after, bring it with you into the 2nd and use it to take out the first 4 beetles so you can keep p-speed throughout

Got it on the second try, not sure why but it seems when I deleted all the saves before I started it broken the achievement.

I picket up a midway gate by accident on the second level, and then started over just to be safe

The achievement must be achieved in a single game session? Because I have managed to overcome the fourth castle and the achievement has not come out, I did it in two days ... could that be why?

I don't understand why I can't get this achievement. At one stage I got a mushroom but they killed me and I did it again correctly, could that be why? Not a single mistake is allowed?

Yep, not a single mistake, I did on my second try, you can't get any powerups or midway gates on the way and beat castle 3, then save after it for the part II, I had to reset when I failed, getting a mushroom, ride Yoshi or anything like it is reset!

Suggested simplification for this achievement in order to avoid forcing the player to go hunt for descriptions: "Starting from Yoshi's house, beat Lemmy and all levels along the way without Yoshi, powerups or !-switches."

The answer to most these questions are literally in the description under the achievement. If people would read before commenting it would eliminate alot of confusion.
