If you get frustrated getting this achievement, try this method.Use the Ultimate Armor code, then fire the Enigma and make it destroy the Colony manipulating the RNG (Check this video, it explains it pretty well: https://youtu.be/mfD7ez0aejw?t=230)

My method: New Game with Ultimate Armor Code (if you're proud, you can forgo the code and get your armor later, but the first 2 stages will be more difficult). Complete intro stage, save game. Go back to title screen and do the RNG manipulation so that, next time you load a save, the cannon destroys the colony on the first try. Fire the cannon. Virus Stage 1 as Ultimate X, gain Rank A. Virus Stage 2 as Ultimate X, gain Rank SA. Waste 6 in-game hours on any Maverick Stage (1 hour passes every time you lose all your lives and return to Stage Select). Go to Virus Stage 3 and get Black Zero and Ultimate Armor (if you don't have it already), kill yourself and return to Stage Select. Grizly Slash as Black Zero, gain Rank GA, select Weapon & Energy + to get Hyper Dash (equip it when avaiable). The Skiver with Ultimate X, gain Rank GA, select Weapon & Life + to get Jumper (equip it when avaiable). Dynamo 1 as Black Zero, gain Rank PA. Mattrex as Ultimate X, keep Rank GA. Squid Adler as Ultimate X, keep Rank GA. Izzy Glow as Ultimate X, keep Rank GA. Dark Dizzy as Ultimate X, gain Rank PA. Dynamo 2 as Black Zero, gain Rank MMH. A few things:
- This will NOT work on Easy Mode, despite that being unspecified.
- For PA -> MMH you basically need the Dynamo Fights or Virus 3. One requirement of MMH is beating a stage in less than 1 minute, which is theoretically possible on Rosered with Hyper Dash but very hard.
- Ranking System Guide: https://pastebin.com/cnvAww69
- UAX is allowed, so it's recommended to breeze through Virus 1 and 2 first. To do this, save after the Intro Stage, quit to Title Screen. Let the Dinorex Demo on the title screen (the third one) play in full, then load your save and fire the Enigma.
- Once X is MMH after Dynamo 1, complete long stages first so you can rank up from GA to PA with Zero reasonably later. I recommend not doing Whale and Dinorex at all. I used Kraken to rank up to PA but there are definitely better stages (Rosered, possibly Pegasus?).

I followed @swordlunge 's guide and it worked. There was just one weird thing at the last Dynamo fight. I finished in less time than him and less than 16 damage but I didn't get MMH for some reason. Then I fought him again and got MMH. really weird

Maybe you didn't get the MMH because you didn't fulfill the max damage requirement on the Dynamo fight. The same thing happened to me until I hit Dynamo once with his weakness

Thanks for the guide swordlunge! i did most of it in slow mode but the last rank MEH as X must be at normal speed
