Something called an evil flag can be active depending on factors including total time played. Do this before 99hrs on clock just to be safe.

You can actually skip the evil flag by using Turbo Fire. You need to have the first two parts of Cait Sith face and when you're about to get the last part of the face, Turbo Fire it so it skips the slot selection and gives you Game Over instead of Death Joker. Watched tons of video, tons of explaining and still win it just by trick with retroarch pause (not in game pause, wheel continue spin).
Check the picture of Slots https://finalfantasy.fandom.com/wiki/Slots_(Final_Fantasy_VII) , then use programm pause to predict Cait face and earn this fkng cheevo -_-

To add to what DEATHDragon said, what you can do is make your accept button your turbo button (X for me). Stop the third reel on the Cait Sith before the face and press accept as you let go of the hold button (square for me) and it will resume the reel and turbo accept in one press. Worked first try. I tried having say R2 as my turbo but the turbo has to be in sync with the accept button and that's what gave me this idea. Obviously don't forget to take it off afterwards.

Once I got 2 faces the above strategy got me emerald weapon kill first try. The tactic only really works on the 3rd reel as you want to cancel the limit as fast as possible. So try and get the first 2 faces via other means, and then get the third by using the accept button as your turbo button.

for those who want this achiev, i found the most easiest thing do 100% chance and not needing those hold buttons (to clean battle screen)

As Cait Sith, at Limit Break Selection (but not showing the slots yet) Press the button to use Limit Break (Ex: X Button). so it will be

X
Wait 500 MS and press X (It will select the first face)
Wait more 500 MS and press X (It will select the second face)
Wait more 550 Ms and press X more then 1 time and it will always use Game Over)

i tested already 10 times and worked always

Heindalr's method turned out to be reliable for me for the first two faces, with some practice. For the third one, I used the Slot images and memorized that it's Bar -> Crown -> Moogle -> Face, and mashed twice when it's around the Moogle/Face area. Although it landed on Star for me, it recognized the input and got me my Game Over.
