I don't understand why it doesn't trigger. I just passed it 3 times ... Are there other conditions? (without using diazepam, without ration, without reset / continue after missing a shot, etc.)?

No, there are no other conditions. I really hope this one isn't broken, it'll be a pain to fix if it is. Is there any way you can record a video of you doing it to show me how youre doing it?

In my first run I tried this again and again without sufficient diazepam, so I tried inventory buffering (spamming L2/R2) to try and make sure each shot hit. Turns out if you open your menu between you shooting and the bullet hitting, as the achievement works by timing the period elapsed between the button press and the hit collision, it'll fail the attempt even if the shot would eventually hit. Very annoying.

If you don't mind waiting or fast forwarding a bunch, here is how I did it. On the southeast corner, lie down with your PSG just south east of the ammo box in front of the slope. You should have a good view over the two northwestern trees, and Wolf will only be able to attack you from there after taking cover behind either tree, which gives you ample warning.
Too close to the slope and you won't see them, too far from it and Wolf will be able to hit you from elsewhere.
Now you just have to wait for her to show up and take your shot when she peeks to shoot from behind either tree. Both are rather easy shots. If she moves east, just wait. It's the best opportunity you have to reload the PSG and recentering your aim.
Save beforehand and don't be greedy with your diazepam, but if you're low on it you might want to take your first 1~3 shots since missing those won't be as punishing.
