if you must spend only 12 bullets, why the Bandana is mentioned? This doenst make any sense

Because the achievement counts the bullets that are fired and if you use the bandana it can't count them

I can't seem to get it, was it because I used save states to restart the fight?

I'm not sure but unless you have them disabled, a small challenge icon should appear at the bottom-right of the screen while the challenge is active. If it's still there, it should trigger upon defeating Ocelot. If it never shows up I would suggest dying and continue to restart the fight. Or save and reload

You definitely have to use "Run n gun" technic, to win this fight (hold X and Square). Shoot, do a 360 round shoot again and repeat until end. Got it in first try

I just dodged Ocelot's bullets and hit him when he was reloading. Not the fastest method but it got the job done. And it is my first time playing this game so that's not bad.

If you don't mind getting shot a few times, simply equip your rations, position yourself right in front of Baker at the bottom of the area, then hold Square aiming at Ocelot across the room. Snake will follow him thanks to the auto-aim mechanism. Whenever Ocelot gets out of cover, the laser will move on the left or right of Baker, meaning Ocelot is in sight ready to be shot.
