If you save at the entrance of the fortress, you've already screwed it up. The bulbins at the towers in the desert map are included in this achievement, and as far as I can tell, doing it at night is absolutely required. If you enter the fortress map during the day, the first tower bulbin will spot you upon entry, without any opportunity for input.

no that doesn't seem to be the case for me - the first tower bulbin doesn't void the cheevo despite shooting at me. the indicator only goes away when getting spotted by any of the others.

I saved in first at the entrance of the fortress during the day and after some fails I saved again after waited the night came and did this achievement without any problems when mostly all of those bulbins took some zeds but don't save just after the alarm has been set, don't know what happens but I don't recommend that at all.
