Seems to be still bugged. I took no damage the whole fight but did not get the cheevo.

I saw that somewhere in phase 2 the icon at the bottom right just disappeared...

Unfortunately I don't have a save anymore from right before the fight.

Two infos I can give here:
- It happend not on the first try. I for sure game over before.
- It did all the damage in phase 2 with jump slashes.

I can retry and provide a save a lot later when I do the 3 heart run. So maybe someone runs into this before.

but the achievement says you can't use the sword after the second form, if you did jump slash you automatically used the sword

okay so I've tried, bow, spinner, boomerang, rolling with iron boots, bombs and wolf form and I still don't understand how to beat it without using the sword ?

Don't just use regular arrows, bomb arrows do way more damage. I had 45 arrows when starting this fight, and finished with 26 arrows left.

Make sure to not swing your sword at all, even if it doesn't hit the boss it will still fail the achievement.

this one took me a ton of tries for some reason. the first phase was basically pure luck, and for the second phase i used the "Fast knockdown backup setup" from here:

Wow, this boss was more complicated than I imagined, I had to repeat the first phase too many times (pure luck), and in the second phase I couldn't get the method that @Cascodius mentions above.

The amount of comments in this cheevo made it look so hard when is actually very simple... you don't need speedrun strategies, just mild reflexes and some bomb arrows

Just do the 2nd phase normal minus using the sword. No reason to do the speedrun strats

The 1st phase actually gave me the most trouble. Pinball really isn't my forte. When I managed to get to the 2nd phase, I did it first try, no speedrun techniques needed. Great cheevo IMO.
