I just got that achievement on duckstation, what I did different is I was saving on a second empty memory card, maybe that helps. If needed: I didn't used the Eagle but I did used the Enhanced ammo, only died once at the mandatory Nemesis fight on Clocktower (still counted for the "one session" requierement). For Carlos I also only used the Handgun, even if it says you can use the Rifle too.

Can you pause the Emulator (Either Retroarch or Duckstation) or you can't stop once?

If you pick ammo like Shotgun Shells or Pick up the Grenade Rounds that Carlos Gives you, do you have to start over or not?

holy molly, I had to take the minethrower to get a file from a dead mercenary. Didn't trigger for me, I think that's bc I take it, even I do not eqquiped.

I can happily finally dispel any rumours about this achievement. You can indeed get it on a ng+! You just -cannot- pick up any weapons, so if you're doing a Jill's Diary run on top of this one pick up the file with Carlos as he can't pick up the mine gun. Also make sure you haven't played mercs and unlocked any inf guns, and don't create any enhanced pistol rounds (just mix 3 Gunpowder A's to avoid any accidents!)

Enhanced ammos for pistol are allowed. Its a 100% checked information. Because i killed final Nemesis using them. And, guess? I get this cheevo -_-

Successfully unlocked on Duckstation in case there's any doubt still. Used both pistols and both ammo types. Ended up doing it without dying so idk if re-loading a save file would actually work.

For those who have doubts: you can use the Handgun and the Eagle 6.0 and switch between them at any time. You can switch between normal ammo and the handgun's + ammo, you can destroy enemies with barrels and traps, you can save and use the machine gun with Carlos. However, it is important to note that it is in "one session" and this means not pausing with the emulator and not resetting the game, you can save, die and load but you cannot stop this session = turn off the video game.

YOU CAN PAUSE THE GAME WITH THE INGAME PAUSE AND WITH THE EMULATOR PAUSE, YOU CAN USE THE EAGLE 6.0 AND THE ENHANCED PISTOL BULLETS, YOU CAN LOWER YOUR GAME WHILE IT RUNS IN THE BACKGROUND. É permitido equipar ambas pistolas (Handgun inicial e Eagle 6.0). Achei mais fácil terminar só com as pistolas do que com um arsenal inteiro, além do gerenciamento de inventário melhorar, as armas são práticas e versáteis.
Recomendo pegar a Eagle o quanto antes, e fazer munição especial com a combinação BBA.
Pode jogar com ambas armas sem crise, só n pegar outra arma pra n cair na besteira de equipar e perder o cheevo.
Não pode fechar o game de maneira alguma, pausa no emulador é permitido. Pode parecer difícil mas é um cheevo mto tranquilo.

Be careful when choose the last action after the final Nemesis battle. Don't choose "Exterminate the monster", this will cancel the achievement because of the magnum.

I've found something interesting.
If you choose to "exterminate the monster", this achievement will be canceled. But the same doesn't happen to the knife-only achievement. In the knife-only run, you may exterminate the monster and still get the cheevo, for some weird reason.
