Did this achievement have to complete the missions until reaching Last Requests in 50 minutes? If so, I did it in 40 minutes and it didn't enable. I have void from my live to show.

Do I have to beat all the story missions prior to Last Requests to trigger this achievement or can I skip whatever I can as long as I complete that particular mission?

To clarify, the missions required to unlock Last Requests are:

-Luigi's missions right until Drive Misty for Me, which unlocks Joey's missions
-Joey's missions until Cipriani's Chauffeur, unlocks Toni's
- Toni's until Salvatore's Called a Meeting, and then after, both Triads and Tribulations and Blow Fish to unlock Bomb da Base acts I and II (you should have enough cash to pay 8 Ball without doing any extra stuff)

Beat Last Requests and the cheevo is done.

Using taxi/ambulance mission skips cancels this achievement. The missions have to be completed naturally.

Couldn't you say that about literally every achievement that is missable lol? All of them can technically be tried again by 'simply starting a new game', thats the definition of missable isn't it?

haven't attempted this yet so real talk, how difficult it actually is to meet a 50-minute time limit if one's decently familiar with the missions? do I need to look into speedrun strats in any detail, or is it doable through more "normal" gameplay at a decent pace?

i feel like I need to prep myself mentally somewhat if it takes like 40 minutes before I can even tell if I should restart a run or carry on

@artanonim if you reach Bomb da Base act 2 on the 40 minute mark, you are good. I made it with almost 4 minutes to spare.

It should go something like this

Luigi: 5 minutes
Joey: 14 minutes
Toni pt 1: 25 minutes
Salvatore: 30 minutes
Toni pt 2: 40 minutes
Salvatore pt 2: 47 minutes

Not hard, BUT still complete speedrun of 1st island.
I'd reccomend to watch old PS2 speedruns for this one, for missions you're not sure you can do fast. Also for setups in terms of guns you'll need. I've used Huge One's https://www.youtube.com/watch?v=yrV05X5v_mE - this one is old and doesn't use any hard strats, but still has some tricks. (The one with adrenaline pill for "Bomb The Base" was extremely helpful for me, cause there are no achievments for passing it intended way in Core set, and also it has quick and easy setups for all missions you'll need).

If you manage just to not fail any of missions and land all the strats (quickly stealing Securicar in Van Heist by blowing up car in front of it, getting Curly Bob in your taxi, Killing Triads in "Triads In Triangulations" from outside, and doing Bomb The Base sniping with adrenaline pill to reduce risk), no matter what RNG you'll get, you'll have plenty of time.

For example: With kinda lots of small mistakes, and tonnes of bad driving I still had like 7 minutes to spare at the end on 1st try.
