Not exactly sure how this is supposed to work since no upgrade on this level gives 10 points and you get a minimum of 200 if not more for killing the enemies.

Okay so the trick was to either 0 your score at 6-1 or enable player 2 and get 2 bomb upgrades, the item is there once you start the level.

The secret is to leave the score with 20 points.
My 2 recommendations:
1- With player 2 in phase 6-2, get the bomb upgrade in this phase, then get the bomb in phase 6-3.
2- With player 2 in phase 6-1, get the bomb upgrade in this phase, then get the bomb in phase 6-2. Phase 6-3 will begin with the secret item. I believe :)
Note: Each bomb upgrade will give 10 points.

Here what I did: Start stage 6-3 (password 7645) and grab only on bomb. Finish the level and on stage 6-4, keep bombing the blocks until appears one bomb icon. Grab that bomb and the secret will appear.
