Neither Action or Riddle difficulty affects locations or accessibility of Memos. Riddle difficulty changes text for Memos that have clues to puzzles.

Exception here are Extra Memos on New Game+ found in Wood Side Apartments Room 205 that gives you clues on how to reach one of the standard endings not yet unlocked on your save.

There are also some clue Memos that will become unavailable if you have all required key items in your inventory. "Inscription on gallows" in the courtyard of Toluca Prison is inaccessible if you have all Tablets, for example.

Make no mistake, this achievement won't land into your pocket just by keeping an eye open while progressing into the game, unlike "Hoarder" for example. I played the game trying my best to interact with everything and only uncovered 33/40 memos. Make sure you use a guide!

I had to follow a guide so you don't have to, here is "free" 11 notes
-6 in north end of neely street, all of the papers on the ground are different, make sure to examine all of them
-3 paintings in north cell block of toluca prison, all of them are in the same cell
-2 paintings inside historical society building, before the long stairs
