The very first Lying Figure and all bosses are mandatory fights. You can't progress the story without those encounters. Fighting anyone else you meet is optional. "Without a single optional kill" means "kill only the enemies you are forced to kill to progress the story, leave anyone else alive".

Do little cockroaches count? I accidentally squashed one while running. I already fought against the first enemy, the lying figure, and the first pyramid head and I got two enemies in "Last seen in".

Yes, Creepers count. Both Pyramid Head battles and Eddie actually does not increase defeated monster count. So if you're past first Pyramid Head battle with 2 kills reported in RP, you've already failed. Next unavoidable kills are three Flesh Lips in Brookhaven.

You can hurt enemies as long as they don't die.

I have no idea who gets the kill if a monster deals finishing blow to another monster though.

There are six unavoidable kills in the game: the very 1st lying figure where you get the Wooden plank, 3 Flesh Lips in Brookhaven, Eddie and Mary/Maria.

There may be an inaccuracy in the description. In the "Rebirth" ending, the achievement did not open for me. I saved after the puzzle with the music boxes, replayed for the "Leave" ending, and the achievement opened.

@benjaminmcp0, On my save, all the main endings were already beated, but I don’t think this is a necessary condition.

I openned the achievement with the following statistics:
Action Level : Normal
Ending : Leave
Saves : 4
Defeated Enemy By Shooting : 3
Defeated Enemy By Shooting : 2

The rest of the stats, I think, don't matter.

I would like this achievement if it wasn't for losing runs to stomping a creeper when I wanted to open a door. Redoing the start of the game is so tedious with all the running around before you start the apartment.

Conseguí el logro usando el Hyper Spray Verde, una vez que derrote al primer lying figure fuí en seguida por el Spray, el cual utilize solo con jefes. Además, el utilizarlo, provoco que no me contará varias muertes de jefes, en mis estadísticas solo contó 4 muertes de las 5 o 6 que son obligatorias.
Por último, recomendaría guardar en solo dos ocasiones que podrían ser necesarias, la primera sería al entrar a la Sociedad Histórica de Silent Hill, ya que, después de avanzar un poco, te encontrarás con una habitación llena de Creeper y puede que por accidente pises uno, y el 2do guardado sería en el Hotel, antes de colocar las cajas de música, ya que puede que no te cuente algun final que saques, de ser así, utiliza ese guardado para sacar otro final.

How to pass the place in the hotel where rooms 204/202 are in the corridor without killing abstract daddies? They just block me and kill me. Because of them, the achievement was disrupted, because with each revival, 1 is added to the save.

Save before and going out to the corridor stick all to the wall so that they stick too and pass sideways. Also, you can shoot them to throw them and pass sideways, it is not necessary to kill them.

@Gubila It's strange that you didn't get the achievement with the "Rebirth" ending. I actually got the achievement with that ending, and it was the last one I needed to complete the four endings. Could it have been a connection issue? or maybe a bug?

In any case, I'm certain it's possible to earn this achievement with the "Rebirth" ending.
