I'd actually recommend going for this on a later playthrough started from scratch, not the first time around (especially if you never played the game before). Most of these conditions are very easy to fulfill, but to clear the game with less than 200k points (basically don't kill too many enemies) you have to really know what you're doing.

I wouldn't recommend playing that way if you grew up with the game and know it well, which is why I advised it in the first place. The game loops after beating it, where you can carry over your stats for a normal playthrough, so it is designed to have a true pacifist feel the very first time when attempting it.

Slight clarification on this ending. You must not go over 200,000 points by the time you finish 3-4. Going over this number of points during the final stage cannot grant you the health required to veer off of the worst ending.
