okay I'm missing something, I just beat the game for a 3rd time, once on normal and twice on sick and I'm not getting the alternate ending

it seems that either this isn't unlocked on sick difficulty or my backup saves were screwing with the progress detection

The absolute joy of breezing through the game on normal to get this achievement after torturing yourself 100%ting the game on sick. I didnt even hit the save button lol.
achievement pops as soon as chapter 27 begins.

I think you have to do normal first and sick second to get the alternative ending. Worked for me anyways.

When you do your second playthrough, do you use the same character or can you use a new one?

Eric Sparrow got what he deserved, both in this game and in THUG2. Jerk.

Did this one on my second playthrough on Normal Difficulty, after completing the game with all goals on Sick Difficulty on a separate custom character.

As spookerson said, you just need to reach to the final chapter, difference is that you don't even need to do the goal to beat Eric's best line a second time.

I have 2 separate save files for story mode, one I've done everything on sick mode and one I'm doing on Normal, do I have to have 1 save file to unlock the hidden ending?

i beat it first on sick, then created a new skater and beat it again on normal and it popped.
