There are three lives capsules in the first part of the level, and then four more in the second part (two in the way to the big launcher and two more in the area below the big launcher where there are a lot of rocket launcher) plus the 100 rings that's the eight lives without going into the third part (which i think also has at least one live capsule)

It seems having 99 lives before starting this level can bug this achievement. Make sure you have at most 91 before starting.
