Is this for only the visible pokeballs (items) on the ground? I back tracked through each area twice crossing out each item there and can't seem to get it to pop. Not sure if something other than those is also a factor.

The items that count are only the ones that are Pokeballs in the overworld. I double checked the logic and everything seemed in place to me. Try pressing Esc and scrolling down the achievement list to the achievement, it will show you a progress bar.

THANK YOU Maxialvarez94! I was scouring the internet for a list of items in SoulSilver and came here as a last resort... The only 2 items I was missing was indeed the two you surf for in Violet City!

I also want to mention, this achievement does NOT include the visible items at the bottom of the Lake of Rage. I never knew the tide goes down on Wednesdays only, and still got the achievement.

Is the "Visible Item" in one of the connection buildings in the Ruins of Alph supposed to not count towards these? Currently only missing one and just got that item but it doesn’t count for me.
