Does this have to be done the very first time you enter the Safari Zone, or can you go through it a few times without collecting items?

Doesn't matter. I'm pretty sure you can't do it multiple times, and I did get it anyways.

Gold Teeth are not optional and are required to beat the game so if I come back and can't get this on my blue playthrough i'll have to do it on my red. Gold Teeth are not optional though. (Ignoring acquiring pokemon through trades with strength.)

be weary of HM 03 (both in the sense you need the collectable item and that you need a Pokmon that can learn it).

The best way to do this is to not pick up any items on your first run EXCEPT HM03 Surf. Then on your second run, collect all the balls. There is one hidden item, a Revive on a statue outside the Surf house.
